Epic Urgent & Family Care started administering Covid-19 vaccine in partnership with Cook County Department of Public Health (CCDPH) and Illinois Department of Public Health (IDPH) from January 14, 2021. We are administering vaccines per IDPH priority groups
We have created a few basic Q&A to answer some of your questions and concerns.
Q: How can I schedule Covid-19 Vaccine appointment?
Appointments can be made online at https://euc.setmore.com. If appointment is not available, you can fill out this form to be placed on our waiting list. We will call for an appointment according to the priority phase.

Appointments fill quickly and slots are limited by vaccine supply per local (Cook County), state (Illinois) and federal health departments. While we have capacity to administer more vaccines, we are limited by vaccine distribution and availability. As these are uncertain times, unexpected schedule change and/or cancellation may occur. Please cooperate with us. Our goal is to vaccinate as many people as we can and as fast as we can! We appreciate your understanding. We are doing our part by massive testing (clinic has performed over 10,000 tests) and have distributed almost 500 vaccines so far...
Q: How Epic Urgent & Family Care will be distributing the Covid-19 vaccine?
A: Epic Urgent & Family Care will strictly follow CCDPH and IDPH guidelines to provide vaccines according to priority groups (phased administration). We will update on our scheduling page whom we are vaccinating at that given time period. If you belong to a subgroup that is being prioritized, please bring the following to your appointment:
Q: I am patient at Epic Urgent & Family care. Would I get priority access to the Covid-19 vaccine?
A: Unfortunately, NO. Epic Urgent & Family Care is simply helping the mass vaccination efforts of the US government to fight Covid-19 pandemic. Therefore, we have to adhere to the guidelines laid out by CCDPH, IDPH and CDC for vaccine distribution and administration.
Q: How much is the cost for Covid-19 Vaccination?
A: There will be NO CHARGE for the vaccine. However, to cover administrative overhead, the clinic will bill administrative fees to your insurance (if you have an insurance) or to CARES act (if you are uninsured). Per current guideline, out of network insurance should cover administrative fees. If you do get an automated bill for vaccine administration fees from Epic, please contact our billing department provided on the billing statement. You have the choice of paying $25 administrative fee if you do not want to use your insurance.
Q: Is appointment necessary for Covid-19 Vaccine?
A: Yes. We will not be providing vaccines to walk-in patients. Epic Urgent Care has the right to refuse vaccination even after the appointment if the appointment is made by mistake and if you do not fall under a given category.
Q: Which Covid-19 Vaccine I will receive?
A: It depends on the supply from the CCDPH. However, you will receive the same vaccine for both doses. Moderna vaccine EUA can be viewed here and Pfizer vaccine EUA can be viewed here
Q: What are the contraindications for Covid-19 Vaccine?
A: If you have had problems in the past after taking any vaccine, we recommend that you talk to your primary doctor about the benefits and risks of getting Covid-19 vaccine. The vaccine was not studied on children, pregnant females and individuals with weakened immune system.
Q: Is the Covid-19 Vaccine preservative free?
A: Yes.
Q: What is I develop side-effects from Covid-19 vaccine?
A: In event of any adverse event, you should contact your primary doctor or go to the urgent care/emergency room. Epic Urgent & Family Care will be more than happy to evaluate you for any side effects if you do not have a primary care doctor. However, that would be a billable service and not a part of Covid-19 vaccine visit. You may also use V-safe from CDC to report any adverse events and get vaccine reminders.
Q: What about the second dose of the vaccine?
A: You will get an appointment for the second dose after receiving the first dose, typically 4 weeks after the first dose. But it depends on the type of vaccine you may receive. Our goal is to get both doses of the vaccine to everyone who receives the first dose at our clinic!
